Group Medical Insurance is becoming more complex all the time with the passing of national and local legislation. And then there are all those confusing acronyms: HSA, HRA, PPO, HMO, PPACA, and GAP. What does it all mean? Where does it end? Let us help! We have experience with all these programs.

In order to obtain underwritten quotes for groups with 2 – 50 employees, we will need a census form completed with names and dates of birth for employees and dependents. If there are current group benefits in place, it is helpful to have copies of the current benefit summaries.

For groups with 50 or more employees, a group health statement with two years of claims experience needs to be completed by someone in Human Resources. If the group does not have two years of claim experience, then each employee will have to complete a Uniform Medical Assessment Form (UMAF's) in order to obtain a group quote.
