In order to obtain underwritten quotes for groups with 2 – 50 employees, we will need a census form completed with names and dates of birth for employees and dependents. If there are current group benefits in place, it is helpful to have copies of the current benefit summaries.

For groups with 50 or more employees, a group health statement with two years of claims experience needs to be completed by someone in Human Resources. If the group does not have two years of claim experience, then each employee will have to complete a Uniform Medical Assessment Form (UMAF's) in order to obtain a group quote.
